Released: Self-Certification of Homelessness Form (Version 1.0)

Overview

Purpose

The Louisiana Balance of State Continuum of Care (LA BOSCOC) has released its Self-Certification of Homelessness Form (Version 1.0).

This form can be used with participants to self-certify their homelessness status.

Note that using this form is optional; while this form was developed in partnership with technical assistance and LA BOSCOC service providers and, when used correctly, meets the standards of self-certification, projects are welcome to continue using comparable forms at their discretion.
