Forms and Workflows

The LA BOSCOC maintains forms and workflows to support projects that provide housing and supportive services to people experiencing homelessness.

All forms and workflows are optional but encouraged unless otherwise specified by the form/workflow or by CoC staff.

Forms

Coordinated Entry Request for Referral Form — used by any housing project (transitional or permanent) to request a participant referral from the LA BOSCOC’s Coordinated Entry Prioritization List. Required for CoC Program and ESG projects. Required for some PATH, SSVF, GDP projects; consult your program manager for more information.

Domestic Violence Prioritization Referral Form — used by domestic violence services providers to enroll participants anonymously on the LA BOSCOC’s Coordinated Entry Prioritization List. Updated 9/2/22. Required for CoC Program and ESG projects.

HMIS Release of Information — used to secure a participant’s consent to have their information entered into HMIS. Secures privacy for all information not protected by HIPAA. Required for projects that use HMIS.

Participant Triage Tool — used by Coordinated Entry Access Points, including outreach providers, to assess a participant’s level of need for the purposes of Coordinated Entry prioritization. Required for Coordinated Entry Access Points.

Permanent Supportive Housing Documentation — may be used by Permanent Supportive Housing projects to verify a participant’s Chronic Homelessness status and to certify a participant’s disability.. Use of these forms is not required for CoC Program or ESG projects. A project’s equivalent form my be utilized.

Release of Confidential Information — used to secure a participant’s consent to have their information entered into the statewide Homeless Management Information System (HMIS). Secures privacy for HIPAA-protected information. Required for projects that use HMIS when working with HIPAA-protected information.

Documentation of Homelessness — used by projects or participants to either self-certify or third party document episodes of homelessness.

Workflows

APR Submission Process — a guide to submitting CoC Program projects’ mandatory Annual Performance Report (APR) to HUD. Required for CoC Program projects.
